Our forms are now available as fillable PDFs. All forms can still be printed and completed by hand. Not all browsers support fillable PDFs so you may need to download the form and open it in a PDF reader such as Adobe Acrobat Reader  to complete it electronically. You can either print out the completed forms or email them to the appropriate staff member.

Some of these forms include signatures, if you have an electronic signature set up on your device you can sign the form electronically with your PDF reader, otherwise you will need to print out the form to sign it.

If a form requires payment (such as forms for classes or retreats), the form must be printed out and submitted with the payment.
